Vorinostat is offered under the Merck Access Program. Uninsured patients who meet income criteria; reside in the US, Puerto Rico, or the US Virgin Islands; and are receiving treatment by a licensed doctor in the US or Puerto Rico must download the , fill it out, and fax it to 855-755-0518. Note that the form includes sections to be filled out by both the patient and the healthcare provider.
In general, patients will qualify for the program if they have a household income of less than $55,850 for individuals, $75,650 for couples, or $115,250 for a family of four. In some cases, exceptions can be made for patients who have insurance coverage but meet other financial hardship criteria.
